Make out and mail check and signed waiver form to Encaustic Art Institute Office, 18 County Road 55A, Cerrillos, NM 87010 with a notation: “Private Instruction” in the check memo. Also, please be sure to indicate the date of your private lesson either in the check memo or included with your waiver form.
